NSPs in Australia have decided that they needed local exchange network. 56K is still very fast there. E1 are about $15k/month.
The trans-contenental network was just finished last week. AUIX Interstate Peering Network is built from 64kb per AUIX site with a 1Mb port speed. There are NAPs are in Brisbane, Sydney, Melbourne, Adelaide and Perth. Auix services include a uniform grades of service, BGP-4 protocol, primary and secondary DNS, and detailed trouble ticket system.
The AUIX is a reaction against Telestra, the dominate carrier and their aggressive line charges.

Each LEC have their own strengths and weaknesses. PAC*BELL used to have good turn-around, but due to a desire of the chairman to "churn the workforce", then service got much worse. They are recovering. In fact , when SWBT bought them, they were surprised at the size of the workforce and are now agressively trying to address this.
PAC*BELL shortages are usually time-slots and hardware in the COs. They do have enough copper in the ground. Though a couple of folks at the meeting who have had experience contrary to that.
PAC*BELL is looking to DSL to solve the problems they have in some areas where some Cos are so loaded with providing access to the Internet they have no slots for dial-tone. However, PAC*BELL is having to put in new infrastructure to support this.
Are any ISPs using dry pair? PAC*BELL doesn't really have any. PAC*BELL is going to put some in to do DSL.
Some kind of ISP certification program might help. Giving the LECs a list of what we need and push them to do it.
John Curran suggests most of the LECs are trying to provide the business directly. He suggests that folks may have to become a competative local loop provider.
